Physical Activity Readiness Questionnaire (PAR-Q)

This health questionnaire is a common method of uncovering health and lifestyle issues prior to an exercise programme starting. The questionnaire is short and easy to administer and reveals any family history of illness or injuries. Please answer all questions to the best of your knowledge.

Client / Personal Trainer agreement (Single Session)

This agreement ensures that the role of the Trainer to Client and Client to Trainer is clearly appreciated and understood. To keep this program running smoothly, I would like to outline the following:

1. This agreement starts and becomes valid once the payment has been received by the Trainer. By making a payment the Client automatically accepts the following terms.

2. This agreement is valid for a maximum of 1 month after the date of purchase or until the purchased session is completed, whichever is sooner.

1x session = valid for maximum 1month

3. The service fee includes one training session with the Trainer, exercise counseling, and prescription.

* Prices may vary based on the location where the session takes place, depending on factors such as venue rental, travel and other fees.

4. The session is 55 minutes, however, a more flexible length can be established.

5. The time of the session is to be agreed upon between the Trainer and the Client. It is not the Trainer’s responsibility to offer options and be available at times that suit the Client’s availability.

6. No expiry date extension or refunds will be granted, except for medical reasons, which must be endorsed by your physician.

7. If a session is not used within one month after purchase it will be lost upon the expiry of this agreement.


Client Responsibilities:

8. The Client ensures that the health questionnaire (PAR-Q) is always up to date

9. If the Client is late, the session will only last until the time when the session was scheduled.

10. Any tardiness of more than ten minutes or absence without formal notification prior to the session will result in the loss of the session.

11. If the session needs to be canceled for any reason other than an emergency, a 24-hour written notice must be given to the Trainer. Failure to do so will result in the Client forfeiting the session and no payment reimbursement will be granted.


Trainer Responsibilities:

12. A Trainer provides the Client with the motivation, education, guidance, and individual instruction required to achieve their personal fitness goals.

13. The trainer will design a safe, effective exercise program on an individual basis that reflects the Client’s objectives, fitness level and experience.

14. If the trainer is late for a session, that time is owed to the Client at no additional charge.

15. The trainer will maintain an open line of communication throughout the course of service.

Client / Personal Trainer agreement (Block of Sessions)

This agreement ensures that the role of the Trainer to Client and Client to Trainer is clearly appreciated and understood. To keep this program running smoothly, I would like to outline the following:

1. This agreement starts and becomes valid once the payment has been received by the Trainer. By making a payment the Client automatically accepts the following terms.

2. This agreement is valid for a maximum of 3 months* after the date of purchase or until the last session in the purchased block is completed, whichever is sooner.

5x session = valid for maximum 2months

10x session = valid for maximum 3months

3. The service fee includes a chosen number of sessions with the Trainer, exercise counseling, prescription, and access to a custom member platform with educational content on the www.akademy.fit website during the active period of this agreement.

*Within the 3months active period of this agreement, if all the 10 sessions in the block have been used within 41 consecutive days from the day of the first session in the block, the Client is entitled for a +1x FREE session, which must be used within 42days from the day of the first session in the block. This means that the Client could either do 10+1 sessions in total in 6 weeks within 3months after purchase or do 10sessions in total within 3 months after purchase.

Client / Personal Trainer agreement (Monthly Payment)

This agreement ensures that the role of the Trainer to Client and Client to Trainer is clearly appreciated and understood. To keep this program running smoothly, I would like to outline the following:

1. This agreement starts and becomes valid once the payment has been received by the Trainer. By making a payment the Client automatically accepts the following terms.

2. This agreement is valid until the last day of the calendar month of the date of purchase.

3. The monthly service fee entitles the Client to 1,2 or 3 sessions/week on average on every X/Y/Z days during the active calendar month period with the Trainer; and includes exercise counseling, prescription, and access to a custom member platform with educational content on the www.akademy.fit website. The monthly fee and the designated training day(s) are to be agreed on between the Client and the Trainer.

4. The monthly service fee is to be paid on the 1st day of every calendar month until it is canceled either by the Client or the Trainer. A standing order is recommended to be set up to avoid any late payments or misunderstandings. When a payment is not received by the 5th day of the month latest, the allocated slots become available for other customers.

New Clients Onboarding Journey:
4a. New Clients have the one-time opportunity to purchase access to a single training session.
4b. Following the first session, a block of 5+ sessions will be used until the beginning of the following calendar month..
4c. Monthly membership begins.

5. Typically each session is 55 minutes, however, a more flexible length can be established.

6. The time of the session(s) is to be agreed upon between the Trainer and the Client. It is not the Trainer’s responsibility to offer options and be available at times that suit the Client’s availability.

7. Any session can be rescheduled to any other available days and times within the calendar month when at least 24hr notice is given.

8. No expiry date extension or refunds will be granted, except for medical reasons, which must be endorsed by your physician. In case of refunds, a £70 unit price for the number of completed sessions (within the active calendar month) will be deducted from the monthly total and the difference will be refunded.

9. Any remaining and not used sessions will be lost upon the expiry of this agreement.
